A 29-year-old farmer presents to your local Emergency Department (ED) with a bite mark over the upper left calf. He is physiologically stable with a NEWS of 0.
He felt a sharp bite while working on his farm 4 hours before presentation. He noticed a spotted brownish small spider over the bite site that he managed to get off his leg. Over the next couple of hours, the bite site has started to get more painful and swollen.
He mainly presents to the ED to get the bite site checked and get an anti-venom in case the situation necessitates that.
On examination, he is generally well and reports improvement of his pain after Ibuprofen given to him at the triage. The bite site shows a two-fang bite mark surrounded by redness and swelling and was tender on palpation.
