A 66-year-old man is presented to you by the paramedics in cardiac arrest. He was in pulseless electrical activity on crew arrival 10 minutes earlier. The crew have failed to intubate him and report he is not easy to ventilate. You aim to continue application of the advanced life-support algorithm.
