A 35 year old man has been sent to the ED from a psychiatric unit. He had been commenced on clozapine 2 months ago for paranoid schizophrenia. His dose was recently increased.
He has a 2 day history of a fever, sore throat and generalised myalgia. Staff became more concerned 6 hours ago when he developed vague chest discomfort and a heart rate of 130. On examination he has a temperature of 37.7°C and blood pressure of 120/80. His ECG shows a sinus tachycardia.
