A 42 year old man comes in complaining of left sided loin pain for the last six hours. The pain increases to a score of 9/10 intermittently, then subsides, but does not go away completely. Nothing he can think of aggravates it. He has never had previous similar symptoms and is fit and well with only appendicectomy aged 18 in his history. He is on no medication and has no allergies. He took co-codamol earlier but vomited it back shortly after.
His bowels are normal and there are no urinary or other symptoms. On examination he is standing by the trolley holding his left side. He looks pale and uncomfortable.
Observations show:
Cardiovascular and respiratory system exams are otherwise normal and there is no significant tenderness in the abdomen. There are no palpable masses or organs and bowel sounds are present.
