A 74-year-old female with a known history of left bundle branch block (LBBB) presents to the Emergency Department (ED) with complaints of general malaise, nausea, vomiting, and weakness. Her past medical history includes hypertension and diabetes mellitus.

On examination, she appears pale and lethargic, with mild dehydration. An ECG is performed and is attached for review.

  1. Half standard voltage on ECG caught me out, so that’s a take home point and just making sure I get an ECG in those with generalised symptoms.
