A 25-year-old obese female, who is 1-week post-partum presents with gradual worsening shortness of breath on minimal exertion, orthopnoea, pedal oedema and a dry cough. She has no chest pain. She was previously fit and healthy.
On examination, she is afebrile, HR111, BP 110/60, RR 28, SpO2 93% (on room air), Distended JVP, Bilateral crackles, Normal heart sounds.
