A 37-year-old woman with a history of bipolar affective disorder was brought into ED by relatives who were concerned that she was confused and agitated. She had apparently been started on a different combination therapy a few weeks previously although her relatives are unsure of her medications. Her sister stated that she had noticed that she had been tremulous over the past 3 days and had become confused and agitated over the past 24 hours.
On examination she is sweating, tachycardic, agitated, verbally unresponsive, and febrile at 39.4.
