A 77-year-old Caucasian female presents with loss of vision in her left eye over the previous four days, which started as a missing patch in the visual field, then progressively worsened over the course of a day. In the last three days she has developed a similar picture in right eye. She denies any systemic symptoms and has had no eye discharge or redness. There is no history of trauma.
In addition to the visual loss, she has developed orbital pain which is worse when brushing her teeth and combing her hair.
Her past medical history includes Polymyalgia Rheumatica, which is controlled with low dose prednisolone.
She is fully independent with no carers or mobility aids.
On examination, she has bilateral swollen discs and significantly decreased vision in both eyes with normal intra-ocular movements. The rest of her examination is unremarkable.

Thorough presentation of a condition we see occasionally in ED. I have even come across a Charles Bonnet syndrome recently.
