What is the diagnosis?
The x-ray shows a dislocation of the tarso-metatarsal joint (Lisfranc fracture). There is gross disruption of the normal relationship between the base of the 2nd MT and the medial border of the intermediate cuneiform.
What else does there appear to be?
In addition there appears to be a bony fragment between the bases of the 1st and 2nd MTs and a longitudinal line at the base of the 2nd MT which probably represents a fracture.
