A 25-year-old pregnant lady in her first full term pregnancy at 37 weeks with previous miscarriage presents to A&E with worsening itchy rash which starts in her abdomen and has now spread to her arms, back and feet. She does not have any other past medical history. A week prior to this, she received vaccination for whooping cough and anti-D immunoglobulin injections. She does not have any infective symptoms.

She notices reduced fetal movements but has had midwife review 2 days prior who does not have concerns. Her rash is worsening and itchy which has caused her difficulty sleeping at night. Bloods are unremarkable, including normal LFT.

This is what it looks like when you examine her abdomen.
