A mother presents to the paediatric emergency department (ED) with her 3-day-old baby girl, who has developed significant jaundice through the course of the day. She is otherwise well.

The baby was born by instrumental delivery at 37+5 weeks. She is breast feeding and producing wet nappies. She has one older sister who required treatment for neonatal jaundice.

On clinical examination, there is cephalohematoma and marked jaundice.
