A 54-year-old male is pre alerted to the resuscitation room with haematemesis and is hypotensive. On assessment he looks pale. He has a past medical history of gastroesophageal reflux. He is on no regular medication.
Observations:
Blood Pressure- 89/67, Heart Rate -110bpm, Temperature – 37.4 degrees Celsius, Blood Sugar – 5.6, Respiratory Rate -24, Oxygen Saturations – 94% on air. GCS-15/15.
He tells you he is a Jehovah witness (JW) and shows you a card.

The patient vomits a large bowel of blood with clots. He drops his GCS to 12 (E3, V4, M5). Observations are now - BP- 80/67, HR -120, T - 37.4 Celsius, Blood Sugar - 5.6, RR-28, Oxygen Saturation -100% on 15Litres.
Transfusion reactions range from mild to life threatening, and it is, therefore, important for clinicians to be able to assess, investigate and manage these presentations.
