A 5-year-old boy is brought to the Emergency Department by his parents due to refusal to walk this morning. He experiences coryzal symptoms over the preceding days and develops a mild fever and bilateral leg pain overnight. There is no reported history of trauma, rash, swelling, or similar previous episodes.

Upon arrival, he appears to be in discomfort and is reluctant to mobilise. His temperature measures 38.2°C, with all other vital signs within normal limits. Physical examination reveals tenderness over both calves, but full range of motion at all lower limb joints. No focal bony tenderness is elicited.

Neurological assessment, including tone, reflexes, and sensation, is unremarkable. Urine dipstick testing is negative for myoglobinuria.
