A 27-year-old male presents to ED 2 weeks after returning from a 6-month trip to India.
He complains of fever, cough, malaise and abdominal pain for the last 6 days associated with vomiting, dizziness and lethargy. These symptoms have slowly worsened over the past few days.
He reports staying with family in India, eating in a number of locations including restaurants and street stalls.
He recalls drinking filtered water and bottled water. He had one episode of consensual unprotected sexual intercourse with a female partner.
On examination: He is febrile at 39.7, tachycardic (115 bpm), tachypnoeic (24 breaths per minute), and is normotensive. His abdomen is soft and non-tender and he is clinically dehydrated.
