A 53-year-old lady with MS presents with left hip and pelvic pain after a fall. She has difficulty in weight bearing since the fall a few days prior.
Clinical examination post analgesia proves largely unremarkable apart from severe pain in the left hip when an attempt is made to mobilise. A pelvic x-ray is ordered. Blood tests show no signs of inflammation or infection. Her urine hcg is negative.

Useful reminder to do further imaging on patients who are unable to mobilise with analgesia despite no clear neck of femur fracture on x-ray – an occult fracture may still exist
