Case 1

Linda, a 58-year-old woman with known breast carcinoma and bone metastases, is brought by her family to the emergency department (ED) with severe breathlessness. On examination, her left hemithorax is stony dull to percussion up to the apex and her trachea is feels deviated to the right. No crepitations or wheeze are audible.

Her observations are as follows:

• Blood Pressure 80/46 mmHg
• Pulse 120bpm
• Respiratory rate of 32
• Oxygen saturations 84% in room air
• Temperature 36.7°C
