A 25-year-old man presents to the Emergency Department (ED) with history of intermittent fever, flu like symptoms, headache, fatigue and arthralgia for the last 3 weeks.
The patient moved to the UK about 5 months ago from Kenya, where he had been working on a dairy farm. He has no past medical history and is not taking any medications on regular basis.
On further enquiry, the patient discloses that he has anorexia, dry cough with night sweats.
On Examination:
- Oxygen saturation is 98% on room air
- Respiratory rate 18 breaths per minute
- Heart rate; 90 beats per minute
- Blood pressure 124/78
- Temperature 37.9 C
Respiratory, cardiovascular and central nervous system examinations are normal. Abdominal examination with generalised tenderness of deep palpation. There are generalized palpable lymph nodes, with no warm swollen joints.
