A 20-year-old female who has fallen off her horse is complaining of right sided chest pain.
Patent airway, GCS 15, pulse 95/min, BP 126/85, SaO2 98% on air, blood sugar 5.5.
During a secondary survey she was noticed to have tenderness over the spinous process of her 4th cervical vertebra as well as complaining of moderate pain over both sacroiliac joints. A trauma X-ray series was ordered. When the X-rays were reviewed an abnormal finding was noticed.

Got it right the first time only with some knowledge about poikilos and grasping it from the pelvic xray lesions. Interesting case and explanation.
