- Failure to consider more than one cause of the transient loss of conciousness
- Failure to assess associated injuries
- Failure to recognise the importance of increasing frequency of syncope: cardiac causes of syncope are more common in patients with recurrent syncope. Increasing frequency of syncopal episodes can therefore be a marker of deterioration in a cardiac disorder
- Failure to recognise the significance of exercise associated syncope, in a significant minority the cause is a serious cardiac cause that can lead to sudden death
