A 50-year-old male, who is usually well, presents to the Emergency Department (ED) with sudden onset right arm swelling and pain after participating in an arcade game where it was required, he punched a bag at full strength. He unfortunately missed the bag, punching the air instead. He felt initially a ‘pop’ to the area and since then has had swelling to the right upper arm with associated pain.

On clinical examination, his right arm appears as in the image.
