Investigation

Unexplained tachycardia may be a clue. Also look for atrial and ventricular ectopics.
Consider bedside echocardiogram.
Consider use of troponin for evidence of myocardial injury [2]
Persistent hypotension with no clear haemorrhage is also suspicious for myocardial injury (once spinal and obstructive causes are ruled out).
