An elderly chronic obstructive pulmonary disease (COPD) male patient sustaining trauma requires a chest drain for a haemopneumothorax. In attempting to do this you are you are unable to advance the drain more than 10 cm into the pleural cavity. You previously were able to aspirate air without difficulty.
How do you proceed?
