A 12-year-old girl is brought to the Emergency Department (ED) after school with abdominal pain that started 1-2 days ago and couldn’t be controlled by paracetamol. She has some nausea but no vomiting or diarrhoea. She has no loss of appetite, dysuria, or urinary frequency. She complains of being somewhat constipated over the past few days and has not opened her bowels for 3 days.

She has been having periods for the past 4-5 months and her last period was a few days ago. She does not tend to have heavy bleeding.

Her observations are: BP: 90/50, HR: 90/min, T: 36.5, RR: 20/min.

She’s been given ibuprofen by the nurse in streaming and says she feels much better. Her abdominal exam is unremarkable. Her systemic exam is normal.

She provides a urine sample and a dipstick reveals 3+ blood (no frank blood) and 3+ protein. No leucocytes and no nitrites.

She is now pain free.
